Full payment is due, and expected, at the time services are rendered.
Forms of Payment
We accept cash or checks. We no longer accept credit or debit cards. Immunization fees are payable by cash or check only.
Any payment arrangements MUST be made with our billing department prior to your visit. To ensure proper payment, our office will verify insurance coverage and benefits using the information provided to us.
Please remember that your policy is a contract between you and your insurance company! It is your responsibility to know and understand your policy’s benefits and coverage.
Be aware that services provided may not be covered under your particular plan. Please contact your insurance company with any questions concerning benefits and/or exclusions under your policy. You will be responsible for paying any copays, deductibles, coinsurance, and charges for non-covered services at the time of service.
